The NMS steam refund thread got me thinking about this topic
A recent game that made me hate it so much was Star Ocean 5, a lot of it was pretty bad but story was what irked me most and i had pretty strong urge of wanting to break the damn disc.

Also had a cousin who when we were kids whenever he lost a match in perfect dark, bad fur day, etc he would get so mad that he would literally slam his controller on the ground. Good thing we sat on carpets and not tile/wood floorings, else that controller would have been a goner.

there's this little girl in your party that constantly gets kidnapped right in front of your face, and whenever you go save her, 2-3 cutscenes later she gets kidnapped again. And this pretty much happens throughout the whole game
