The point is that it's satire. The picture features multiple deities in offensive sexual positions, yet no one has been murdered.

The image in the Onion article is very extreme in itself. Maybe you check out images like that every day? or draw some yourself? but as a satirical newspaper site that image is hardcore.

If they put Muhammad in it the headline would have to be "Five People Murdered Because Of This Image" which wouldn't be funny because five people did just get murdered over some shit like this.
