The Free Labor quest from Fallout 3's The Pitt.

Went with taking the 'cure'. Can't say I felt too good about it, and to cap it off it was then implied that the place was starting to go to hell without any proper leadership present. When the other option is endorsing slavery it doesn't seem like there's any good choice.

Deciding on a character build ..
Always takes me ages before I make it passed the 'create your hero' screen.

Probably something in The Walking Dead, a few moments come to mind. Maybe (Ep. 4) Lee getting bitten and having a split second to decide whether to hide or show the crew was the most intense, but there's a half dozen decisions throughout the season that also rank right up there. It's pretty much Hard Choices: The Game.

I now have a succession of character names that I carry from game to game for that purpose. It started with a four-male-character FF1 party, then I added in a female name for FF2, and that five-man band was sufficient for most RPGs after that, until Etrian Odyssey.

On EO, though, I had to go a bit ambitious with naming, so after my five-man band I got punny. My Hexer was Exa, my Ronin was Roan. My troubadour was Spuuni, which I've been secretly proud/ashamed of for quite some time. My farming group of survivalists were named after various fictional farmers; Barleymo, Brown, Giles.

I've kept that list of names for EO2, and I'll probably continue to revisit it for any RPG that doesn't have default characters.

KOTOR1 where if you turn Sith on the lost world place, you have to kill Mission. I was stuck on that screen for a long time trying to decide whether it was worth it because while I thought Mission was terrible for combat, she was easily my favorite character.

Most agonizing one was sticking with Tali in ME3 even though I was with Liara in part 1. I don't if everyone's game plays the same, but after telling Liara I was serious about Tali, like tons of dialog and scenes with just you and Liara kept popping up all throughout the game. She makes that time capsule report thing and and you chat on the Citadel looking over the city talking about life and stuff. I was always in those scenes like, "dammit, I made a grave mistake," and hoping there would be rekindle the old flame.

Choosing my name and/or class for an MMO/RPG. Takes me an hour usually because I am just so uncertain. When I first got WoW it took me over 2 hours to decide on a class, reading forums, looking around, etc .. .

I don't "re-roll" unless something goes absolutely haywire so my first choice I usually stick with.

Almost every f2p micro transaction based games:

Do i keep on playing to grind the hell out, pay up or abandon the game once and for all?

Luckily the last option wins every time, and I'm proud of it. As a down side, I pretty much never played a f2p game again.

In GTA4, there is a choice where you can kill one of two people. One is clearly the "right" choice, as he is the one struggling to make his life right while the other guy is a piece of shit. When given the choice of who to kill, I accidentally picked the wrong guy. There was no way to "un-choose" the guy. But I hadn't saved in a while. So the hard choice was, lose 2-3 hours of progress or just kill the guy.

So I killed the guy. Then the game literally yells at you for doing it, and you get nothing for it (I think it locks you out of some missions/bonuses too). The fact that the choice was so black and white really soured me on the game.
