Homura is from XC2. She is an extremely caring person and is mostly softspoken. Despite having an outfit that looks like that you'd never know unless you played the game. Force's entire argument consists of Ellie being the way she is because of realism or her surroundings. I say that isn't the case since realism was thrown out the window in the first 6 minutes of the original game and the design could have been literally anything. That was ND's choice and there is nothing wrong with that. Creative freedom is still creative freedom even if there isn't much creativity in it. Hell her immunity could have worked as a way to make her grow more or a different mutation altogether for all we know! I just found it absurd people actually still throw realism around like that on games like these but whatever.

Oh so it can only be realistic when you want it to be realistic? As if plant zombies are realistic? Or a woman taking down men twice her weight is realistic? Again that goes both ways. People are free to criticize something they don't like. Just like Naughty Dog is free to make Ellie as unappealing as they like. It is their videogame character after all but to think realism or personality is why they made her like that is hilarious to me. It is just their choice more than anything, if they want they could give her a third arm and it wouldn't matter because the game's setting is fucking zombies lol.

You make some valid points about how creativity need not be hindered by a character's traits, I understand that. The thing is though, TLOU is a game with close ties to our reality. It is literally a depiction of what could happen to our world if cordycepts fungi (again, a real infection as apposed to the Z virus or some shit) were to infect us. Our people, ourselves and our society.

Because of that, TLOU can't get away the same kind of things as you can in a game XC2 or Gravity Rush because they are full on fantasys. The look of not just Ellie, but all the characters in TLOU have to reflect their reality, and that reality is our reality, just a version of it were cordycepts infects humans. So realism does matter.

ND decided to show in Ellie's appearance that in the last 5 years this girl was preparing, training, readying herself physically for whatever challages were to come. The result is a fit young woman with a physical appearance to reflect that. She's muscular, but not to the "roided" extent people are saying she is. People take issue with this for no good reason.

I suspect it's not some conscious decision to send a message that women are physically like men. It's not the first or last time models haven't been perfect, and uncanny valley is a thing. They just probably wanted to make a fit female, made a model, it seemed fine to them (as you see, to many people including me, it does), and they moved on to other things.
