Also, any significance that every patient in the hospital is male, with shaved heads? And I can't tell if the car crash scene is taken out of sequence of events, as in it happens later post-hospital.

Shame on you. MGS4 is my 2nd favorite game of all time.
I thought the gunshot to the head was a bit unnecessary. Otherwise the only complaint I had about it (before I realized it's likely MGS related) was that the text interruptions felt out of place. I would have preferred if it was all just the video cut together with no text.
With all the retcons they've done since then, is Metal Gear even canon to the series anymore?

I think PS3/360 are just placeholders, it looks next-gen to me.
I don't know. The only Metal Gear where I honestly care about the story is 3, and even then we have flying hover scooters in Soviet Russia, so I care about it in the way I care about a good bad 60s/70s movie.

i think it may be Ground Zeroes. well, Ground Zeroes portion (breaking in that base and trying to rescue that kid) may be like virtuous mission from mgs3, and then something happens - landing Snake in a coma - and then the main game (MGSV) commences.
so ground zeroes = virtuous mission from mgs3 / tanker mission from mgs2
and MGSV = main game

Reading up on Project MKUltra, I have to wonder if its connected at all, as it sounds similarly fucked up. Has MGS made any reference to it? I've yet to play Peace Walker.
In any case, the hallucinogenic stuff is fascinating.

Yeah, I was quite interested in the game even before the Metal Gear stuff became apparent. A game about a returning soldier who was injured badly in an accident struggling with his own body and his mind (PTSD, the hallucinations of apocalyptic imagery) after going through such a traumatic event. Which seemed really cool. Quite a mature subject which could be really interesting (meditating on violence and war and that stuff...which kojima does quite a bit so I should have guessed) Yeah, I really like the atmosphere of this trailer, I hope we get more of that.
Maybe it's a horror survival metal gear game, those pictures of the 'psycho mantis' floating figures...a horror game set in an army hospital with a severely injured protagonist...woah

may not be vita, but definitely not next gen. poly count for the character model actually looks pretty low for even ps360 standard, which is what start the Vita speculation. I assume next gen is capable to handle a much higher poly model.
