It's strange how the blast initially is just enough to fire the cover upwards, but then a massive second blast is what really does the damage.

Probably didn't have a great oxygenfuel ratio. After the first blast it sucked more air in from other places giving it an even more explosive mix.

Impressive, but not as scary as pouring a small amount of water on a grease fire. FaaaaWHOOOOOOF!

I thought this was pretty obvious and this man was clearly very stupid.

And CH4 isn't the only gas you'll find below a manhole. Confined spaces like that also often have build-ups of H2S due to stuff rotting, along with other nasties like CO from nearby traffic and potentially low O2 levels due to displacement. Be warned.
