The Elder Scrolls IV: Oblivion - Shivering Isles

Bloodborne: The Old Hunters (At least, it is the best Souls expansion)

My gold standard for DLC is still The Shivering Isles from Oblivion.

The Smash 4 character DLC was really good as well. All of the brand new characters have great movesets and they really feel super unique. Ryu with his motions for special moves and extra normal moves, Bayo with her ridiculous combo game, Cloud with his meter shenanigans, all super good characters.

The Old Hunters is certainly the best DLC I have played so far, so my vote goes to that.

It's a step above the already fantastic base game and elevates Bloodborne to GOAT.
 
It's Heart of Stone for me, no question. Nothing else (including Blood and Wine, which is great), comes close.

Perfect size (manageable but still a reasonable chunk of stuff to do), and, most importantly, introduces new interesting characters and finds clever ways to highlight different aspects of the main character than did the main game.
