For anime, pretty much anything by Yoko Kanno, don't have a clear favorite either.

For games, there's a lot of memorable RPG tunes, but I'd say Mr.Driller OST tops the list because you don't even have to play the games to appreciate it.

Joe Hisashi - Everything Miyazaki from Nausicaa. Amazing composer.

Whoever did some of music for RahXephon and Eureka 7, especially the opening themes.

Whoops nealry forgot Hirasaw Susumu - Berserk, Millenium Actress, Paranoia Agent, Paprika

and

Tsuneo Imahori for the Hajime no Ippo soundtrack as well as Trigun.

I am not exaggerating -- the Final Fantasy XIII OST is perhaps the best in the entire series, and without a doubt one of the best game OST's of all time. I have never heard game music this good. Period. No, I'm not jumping on the hype bandwagon as it nears release in NA/EU. It's just a flawless OST. The bar has been raised. I don't know if it can be raised much higher.
