To unlock him in gears 3 you need to get seriously 3.0
To get seriously 3, you need to get a certain amount of onyx medals (medals are given to you for kills, assists, executions, captures, etc, onyx is the last level of medals. For example: bronze headshot medal is like 100, silver is 500, gold is 1000 and onyx is 5000 now imagine that with like 63 more medals.) With a ending of almost 200k kills (after resets) I finally got him, then I realized he was the same character model as a low level unlock character but with a new head

You can actually just enter your name as SPELUNKER to unlock it, you dont even need to play the game at all to unlock it. Obviously you should probably save it for last though, at least second playthrough, since you die in one hit
I was lucky enough to get the 3-pack, but I couldn't believe they hid all of those challenges behind three Amiibo-locks
The squid isn't even available in the US by itself, so tons of people have no way of getting three pieces of unique gear, a weapon, and the squid DJ game

For some reason MGS4's emblems really come to mind. Shit like having to spend hours and hours inside a barrel or whatever. Not fun at all, and not even really gameplay by any meaning of the word. No sane person is going to do anything other than just leave the game running, so I don't know how they thought it was cool to have things unlocked that way.
Two of these requires you know how to get the rewards before even starting the game so at least 2 of these almost no one will even get in their first playthrough.

The ridiculous requirements of characters in Smash Melee after Pichu are just downright silly.
It takes like 2 hours at most to unlock every character except Mewtwo and Game and Watch. And then Mewtwo hits with his ridiculous requirements and Game and Watch requires Mewtwo to unlock.
Meaning it would often be months between Pichu and Mewtwo and another long time between Mewtwo and Game and Watch.
Mewtwo was a myth around my school for the longest time. There was that one kid who managed to unlock him, but didn't know how and nobody else really knew and we all speculated if Mewtwo was even real. We didn't even know who Game and Watch was or if he even existed until, like, months later.
And that's not even getting to the stages. The requirements for some of those are ridiculous as well. 15 minute Melee? Are you insane?

Unlocking all the Knack types sucks because of all the duplicate crystals one gets. Since there are a set amount of chests, you will have to beat the game over...and over...and over to unlock everything.

Retailer-specific pre-order bonuses are the worst. Like, regular pre-order bonuses aren't that bad to me; I don't mind pre-ordering games I want at all. However, I am not going to go around, making multiple pre-orders at different retailers just to get the full game content. The Gamestop-only Red Hood DLC for Arkham Knight, for example, is selling for like $40 on eBay right now. This is ridiculous.

Not really an unlock, but since people are talking about bullshit ways to acquire items in game, pure bladestone comes to mind (Demon's Souls).

Wasn't all that hard because you could just set the game on super easy mode at one round, pick zangief and abuse the lariat.

Do trophies count? Star Ocean 4 Platinum trophy is the hardest and most ridiculousI know of. Requires going through the game something about 5 times iirc, at least twice on hardest.

White Fatalis G rank (along with his weapons and armor) in MH4U, I think is an online dlc event or something. Yeah I'm not waiting months to be able to download that guy, when I know he came with the cartridge. He's probably unlocked right now as I typed this, but I stopped playing the game two months ago and don't really feel like picking it up again.
Catching Spiritomb legitly in Pokemon Diamond/Pearl was a pain in the ass, good thing they can breed. You had to connect to at least 32 real life people in the underground with the Odd Keystone in your inventory. Gen 4 had a lot of shenanigans like that, imagine trying for a Munchlax on a Honey Tree instead of breeding a Snorlax for one instead.
