Demons / Dark Souls might not be traditional survival horrors like Resident Evil, but survivalism is still a massively important component of the game design, and art draws a lot from gothic horror. I'm not sure they're what the op is looking for.

Even though it doesn't necessarily have a "boss" Heart of Darkness, how this game got an E rating I will never know, couldn't finish it though due to WAY too much trial and error gameplay.

Definitely a character that came to mind for me when reading the thread title. The entire act is pretty unsettling.Since I don't class The Last of Us as survival horror, I'd say (spoilers).the rapist/pedophile/cannibal/Nolan North, a.k.a. Dave. It's made more creepy since you combat him while playing as a kid, and he can fuck you up quite graphically. Pretty unprecedented in a video game
