For me it was the stomach end boss of the original Contra. Close second would be the alien head/penis miniboss in the middle of the same level.

Well... probably nobody here has played this but... those freaky looking things from The Suffering with syringes for eyes.

Man, when those guys get ahold of you and you see them trying to force their syringes into your face... ::shudder::

Pyramid Head, he has class beyond dudes with syringes for eyes.
