It's a classic movie scene, the bad guy and the good guy both have guns drawn on each other. In a more interesting situation, the bad guy has his gun pointed at a third target - usually someone of interest to the good guy. The situation doesn't matter, it just has to be a gun standoff.
Main question is - Why don't they just shoot?
Say the good guy just shoots at the bad guy, will the bad guy have a quick enough reaction time to even shoot back/shoot the hostage? Usually in these scenarios they are maybe 20-30ft away from each other. And usually in these scenarios NIETHER of them shoot. This has always bugged me.

Average reaction time of a human - 0.2 - 0.25 seconds
In 0.2 seconds, a bullet will travel about 64 meters, or 209 feet.
