My mind was blown after 15 minutes of fighting Mantis, after trying to interact with every damn desk, picture, table, whatever in that boss room, thinking to myself "where is this goddamned controller they are talking about moving?" Then, "OHHHHHH!!!!"

Sort of off-topic (can be interpreted as NOT breaking the 4th wall), but I think the ending to Final Fantasy Legend is worthy of a mention.

The ending never directly references the player or anything else, but the game hints at the "creator" actually being a game designer. There is also the door after defeating said boss that supposedly leads to "another world". It's a surprisingly meta ending for a pre-Evangelion era where JRPG plots still had roots in traditional fantasy stories.
