Games where changing your appearance affects your interactions with NPCs

A friend and I were talking about BOTW, and we both agreed it was a shame that the
Gerudo outfit
only triggers exclusive dialogue in a certain region of the game, instead of having an effect in the world at large. Yeah, we're aware of how large an undertaking in script that'd be, for something so minor too, but I've always loved when games go the extra mile with details most gamers won't notice.

Off the top of my head, an example from the same franchise:_ I loved how several NPC in Majora's Mask will react differently according to whatever mask you're wearing (same is true, to quite a lesser extent, of OoT), at times just for funny conversations, at others even opening optional sidequests.

The masks are obviously much more inherent to the game design of MM as a whole than the aforementioned item is in BOTW, so hey, help me out digging better examples! I'm sure there's been lots of games that have done this to great effect. Let me hear of them!
 
Fallout series will have an impact depending on the gender of your character, and subsequently certain NPCs will further change how they react to you if you are wearing certain outfits.
 
Get ready for south park 2. Sounds like the entire game does this depending on your difficulty (which affects your skintone) and your gender.

divinity 2 dose it amazingly well playing as my undead necromancer i have to keep covered the entire time to not let anyone see that i am a skeleton man or they will freak out and attack me
i also have to be careful how i interact with npcs as well during dialog choices no shaking hands and stuff like that

You sure? They made it sound like the only thing it effects are how much money you get from combat and dialogue options.

We reached out to the game's publisher, Ubisoft, who had no official statement but confirmed that it's just a joke. It doesn't affect combat difficulty or any other aspect of gameplay difficulty. The slider changes skin tone, but it's purely cosmetic.
 
In Divinity Original Sin 2 you can have party members of different races, and NPCs will react entirely differently.

I walked up to a dwarf with a human and he said "I don't rub elbows with fuckin' humans"

But I talked to the same NPC as a dwarf and he talked to me about a quest or something.

It's a good game.
 
Costume Quest 2 has a costume system that allows you to interact with NPCs and the environment differently, depending on what you're wearing.

I'm not necessarily recommending the game, though. It's short, and extremely average. I only played it this past weekend for the holiday feelings.
 
Hitman - Sneak into areas in plain sight, be able to perform jobs of the person you are dressed as, etc. Basically the entire game is built around this idea.
 
Vampire the Masquerade Bloodlines plays completely differently if you choose Malkavian or Nosferatu . Nosferatu in particular has to do with appearance, as you're considered too ugly to blend in with humans.

You could go from skinny to ripped to obese and people around you would comment on it. In fact, some of the women you could date prefer one body type over the other. I think hair and clothing had an effect too.
 
Super Mario Bros 3 will show different messages from the kings you rescue with certain costumes (Frog, Tanuki and Hammer Bro).
 
In Horizon people often comment depending of what you're wearing. Also there's a place near the end of the game where I think you'll get attacked if you don't wear a special armor.

Vampire the Masquerade Bloodlines may do the most interesting appearance-based reactivity. As Jarth Mader mentioned, if you play as a Nosferatu, your character is too obviously non-human to keep up the masquerade, so you're forced to rely on stealth where other characters would just stroll down the street and walk in the front door.

In Fallout: New Vegas you can get into trouble wearing the wrong faction uniform in the wrong neighborhood.

I have not gone far in the game but no from what I played. While it is cool from what i played so far I think its shallow system, the npc just saw me with some association elf/dwarf/etc companion. It should take some time to be able to gain the trust or speak with that npc again not a simple party switch.
 
Baldurs Gate 2. When you disguise yourself as drow during the underdark chapter, npcs will react to you differently than if you were undisguised. For example the gnomes will be afraid of you and some of the npcs you can rescue during one of the side quests will be confused as to why a drow is saving them.

It's a good game.

Sometimes it's even super subtle, I play lizard folk lady and i had to restart a couple of times, im not sure what it is but one time the Red Prince treated me with respect and the other time he checked out me like some sort of cheap concubine.
 
Megaman Legends 1 and 2 (the sequel moreso)

Doing good deeds and tasks around town give you a lighter, more heavanly look. NPCs and shop owners react more pleasantly toward you, and even give you discounts on items in the store.

If you do bad and generally fuck with people and hurt them or animals, you begin donning a darker tone, and people become short with you. Prices go up, and I think some of them outright refuse to do business you! Like with the “Light” Megaman, certain items were only accessible in this morally corrupt form.

It’s one of the earliers morality systems out there I think. MML1/2 were way ahead of their time.

In Demon's Souls you can only access a hidden part of one area by equipping the Fat Official hat.

You basically you fool one of them and make them think you are one of them

There's quite a lot of this in Destiny 2 depending on your race and class, some environmental scannables will be completely different or possibly not even available. Which I thought was a nice touch that I wasn't expecting.
NPC say different stuff too, but it's usually nothing more then repeated lines they say if you stand around them.
 
in Tales of Xillia, if you walk around Millia's village with her as the active party member, the villagers will bow down towards her as she passes by them.
