Some Lars Von Trier movies, like Dancer in the Dark and Breaking the Waves

For more graphic stuff, check out the french new extreme. Martyrs and Irreversible are some examples that have been mentioned. I'm very partial to I Stand Alone as well. Otherwise there's also japanese movies like Coldfish, Audition and the like.

thats really good, cast of 2 and you are not really sure who the 'bad guy' is
 
I second the Lars Van Trier recommendations. Dancer the Dark and Breaking the Waves in particular.

Michael Haneke's Funny Games (both versions are exactly the same) and the Piano Teacher are tough watches.

If you are fan of reallllllly slow cinema, Chantal Akerman's Jeanne Dielman, 23 du quai Commerce, 1080 Bruxelles, and Tsai Ming Liang's Stray Dogs are two films that left me emotionally exhausted.
 
Seeing Threads mentioned made me think of When the wind blows by Raymond Briggs, the guy that made The Snowman.

Also Hard Candy is worth a look into also. They may not be what you want but i'd say they're worth your time. Most of my other recommendations have already been mentioned.
 
The big ones have been taken care of so these might not all fit your criteria perfectly, but they are definitely bleak and feature bad things happening to people.

Reign Over Me. Don't be fooled by Adam Sandler on the marquee. Possibly his best movie although it's psychologically heartbreaking more than anything physical.
 
Speaking of Requiem for a Dream, the same director also made Black Swan. One of my favorite films and very reminiscent of Requiem's editing that visualized emotions and the context of what the characters were experiencing. Highly recommended.

Lost in Translation could also fit your description. It's certainly realistic in how down to earth it is, and the central theme's will play to any apathy you have of your lifestyle, whether it's through your relationships with people or your own success. Bittersweet definitely defines most of the film, and certainly the ending.

Matchpoint is definitely pretty fucked up. And a great overall story. Kind of a drama but shit goes down.

The Gift(2015, with Jason Bateman) is a great thriller. Keeps you guessing the whole time.
