Should have a topic on that. Personally, I'd be all for a system that allows kidneys to be sold and it'd be relatively easy to made safe and fair. Possibly partial liver donations too.
- A kidney has a set value.
- A kidney must be donated by the donator in the hospital. No loose organs.
- The donator places their name on a list and waits to be called. They can always back out.
- Time of death donations are not payed for.
Basically, keep two lists. 1 for people who cannot buy an organ. 1 for people who can afford it.
This will allow people to make some money if they need it. Move some money from the rich to the poor.
The rich filter off the donation wait list, effectively shortening said list.
People are more motivated to donate a kidney, total supply of kidneys goes up.
Admittedly, there is the possibility that too many people who would donate organs at death would instead sell one early, but it would be moot if we made donation the default rather than an opt in thing. And you'd still get one after death.
