When you as Garcian Smith find out who the fuck Emir is at the end (and you finally get to see what's inside his very long briefcase that he's been carrying around for the entire game). Actually every story part in that game, but I jumped out of my chair and said "holy shit!" when that happened.
In Metal Gear Solid 3,
You find out you've been playing as Big Boss. Ok so this one isn't that big, but I like the way they did it at the end very subtly, "you've been given the title Big Boss."

Both of Ashley's flashback scenes in which his family dies. His son getting arrowed as he ran to him... he was like 7 years old. Couldn't believe that was in the game.
Sydney getting the majority of his back peeled off and realizing he wasn't the enemy.
Guildenstern killing his woman right before the end of the game. Son of a bitch!

Metal Gear Solid 3: Snake Eater -
I already knew the Boss' fate before the end, but piecing it all together, how all of the "betrayals" in the series tied together and were just like this, but The Boss' being the one that stands out. It made me question the next one being...
Metal Gear -
Big Boss' betrayal. Thinking about now, some reconning of that ending. Big Boss wanted Snake to kill him. Same as he and the Boss, the sorrow and the boss, Jack and Solidus.
Xenogears -
The "Soylent Green is people" moment and Elly and co. vomiting.
Final Fantasy VIII -
Making it seem like Squall died at the end of Disc 1 just like Aeris. Not really surprising because I was skeptical even after it happened. :lol
Final Fantasy IX -
The unhappy endings, holy cow. Vivi dying was a big one and most overlooked is Freya finding her love but to not have his memory return then remembering her phrase from the beginning photos of the game, "To be forgotten is worse than death." Most tragic.
Tomb Raider 2 -
The raid at her house and the end!
Tekken 2 -
Kazuya's ending. Wow, never in a million years would anyone expect Heihachi Mishima to sacrifice himself to save his son from Devil. It was a very touching moment. That is, until Kazuya realizes he can kill two birds with one stone and uses Hei's body as a shield and rushes Devil with Hei's body. What a family of bastards. :lol
Mega Man X -
Finding out Dr. Light and Megaman died and they play that really sad music at the upgrade spots.
Soul Calibur -
Mitsurugi's ending. He charges a group of gun-toting men and is "never heard from again." The tragic hero.
The Revenge of Shinobi -
The ending where the princess dies if you take too long. I got this ending most of the time as Zeed is a CHEESY PIECE OF CRAP boss.
Gungrave -
The scene in the elevator where you learn most of the story. Harry betrays Brandon and shoots him in the damned eye.
Zone of the Enders: The 2nd Runner -
Leo is piloting the friggen Vic Viper and tries to take back Jehuty. Also, right near the end where you receive the Zero Shift ability. That thing rocked.
Halo -
The Flood really surprised me. No, not the moment they appeared, but the moment I realized they were going to be the main enemy for the rest of the game.
Halo 2 -
Playing as the Arbitor and noticing Keith David's voice right away. Playing as Goliath rocked. Also, the tease ending and realizing there wasn't anything for beating it on legendary sucked, too.
Yeah I know, thats why I said it wasn't that big a deal, I just liked how he got the title at the very end of the game, though it wasn't as revealing as the other revalations in this thread. There are probably better examples in this game.
I'm honestly thinking back all the way to MGS and I can't think of anything that had a bigger "OMG" moment than KotOR. It was so nicely done and fit well with the original SW trilogy.

Palom and Porom turning them selves into stone to save the rest of the party.
FFVI -
The entire opera scene with Celes.
Ninja Giaden (NES don't remember which #. Think it was 2) -
Just as you get to the final boss you see him kill the girl you've been trying to rescue. This was especially big because before then I had never seen anybody die in a game who wasn't a bad guy.
Castlevania II: Simon's Quest -
Dying at the end. Even though you defeat Dracula if you take enough damage during the fight you "Never recover from your wounds" or something like that.
Freespace 2. You finally think you beat the Shivans biggest starship and all is well.
Then 80 of those big ships start comming into the Capella star system and start converging around the star .
Then while evacuating civilians from the system, suddenly the star goes supernova, you fly as fast as you can to the jump point, but unless you know it's comming it's very hard to make it, i died many times(i think you got a different ending depending if you make it out or not.)
I remember being so amazed watching that star expanding toward me, knowing i couldn't do a damn thing. You realize that the Shivans are totally unstoppable and totally unpredictable.
This may not be what you're talking about, but I liked "Star Road" in Super Mario World. That was a nice surprise, and for some reason felt like something special beyond the regular game.
KotOR was a big surprise. Unlike some other people, I didn't see that coming.
The Flood in Halo really got me. The first encounter with them was really freaky in a "What the heck?!" sort of way.
Zelda: Link's Awakening was another one, I suppose. I played that a long time ago, though, and I don't remember whether I knew the truth already or not.

ICO comes to mind obviously since it makes you feel dirty, and I really liked SotC "reveal".....
EDIT: ... that was the suprise in KOTOR? ... I thought that was the gist of the game, never really thought otherwise.

While I preferred ToS overall, Baten Kaitos had one of the craziest twists I've ever seen in an RPG and that's really saying something. I still wish it would have been explained a bit more, but when it first happens, your jaw just drops. I mean I usually have a decent idea of what's coming next in a game, even with MGS or FF games. The big twist in Baten Kaitos is the most shocking moment I've had in a game in a long, long time.
The surprise in Oddworld: Stranger's Wrath turned a fun game into one of the best of the year. Great little title, and the only exception to my "don't give money to EA" rule this gen.
Wing Commander 3, when your tiger friend (I forget his name.... we was your wing man in the first 3 wing commanders) turns on you and you have to go after him.
