Special mention to Theif 1, Deus Ex, SotC, Kingdom Hearts 1, FFXII CE, and the original Super Smash Bros. case. I can't think of anything else off the top of my head.

It's my favourite boxart of the last console generation. It's a simple cover done right.

The best version of that boxart is the one used on the soundtrack.

Obviously the tri-goggles are the most recognizable thing about Sam Fisher, his face changes in every game and even in the games themsevles the character looks nothing like the one in the CG cutscenes. It's one of those characters where the face means absolutely nothing. So, why would you want to put his face on the cover?

The boxart itself is pretty simple and it perfectly sets the mood for the game, you have the tri-goggles lurking behind a glass window that's covered in raindrops and the single stream of blood flowing down from the top. It shows that Sam Fisher is not your usual action hero, he kills silently and he sneaks around his victims, waiting for the perfect moment to kill them.

Look at the Halo boxart for comparison.

What does it tell you?

What kind of game are you expecting?

You'll expect a game with lots of action, with a big green cyborg who kicks ass, big battles, vehicles, lots of bright greens... blablabla

The Chaos Theory cover shows exactly what to expect in the game, there are no guns on the cover, it's dark, there is no smiling hero, not even a big splash of blood against the glass window, just a slight hint that something horrible happened there.

But that's just me, and the other guy who suggested it, it seems:)
 
Looking over this thread, it has become clear to me that game package design hit its peak with late 80s/early 90s PC adventure games...
