Yeah, if Tony dies, it'll be to either save Peter and/or Steve. I lean more towards Tony surviving because I feel like Marvel will want to keep the door open just enough so that they can bring RDJ back for brief appearances if they want, but it wouldn't surprise me much if he does die either here or in Avengers 4.
However, despite Chris Evans' enthusiasm for the role and willingness to appear in future films, I just don't see Steve surviving Avengers 4. Between Evans' current contract ending there while Sebastian Stan's contract for Bucky having wayyyyy more movies left on it, as well as how Steve's personal arc has been progressing over the course of the MCU, I feel like it's all building up to him sacrificing himself to help bring down Thanos and passing the mantle of Captain America down to Bucky.

I know I'm playing into the whole Marvel "stakes" trope, but given the power of the gauntlet, can't ANYTHING happen and then be undone?

Yes.
I mean in theory this could all end with everyone on Earth being dead, then Cap or someone uses the Gauntlet to reset the Earth to where it was even prior to Phase 1. Then we get a full-fledged reboot of the MCU.
I don't think that'll happen, but it's possible in theory.
That's what happens in the comics. And it's one of the best comic stories ever.
