32% of gay and bisexual men in Washington, D.C. are infected with HIV

More than a quarter of gay and bisexual men in some cities and states in the South are living with H.I.V., according to a new study — a far higher rate than in the country as a whole.

The study shows how much more common H.I.V., the virus that causes AIDS, is among gay black men, especially in the South, as well as how little is being done to prevent its spread in a group whose members face discrimination and are less likely to have medical insurance.

Gay men with insurance are more likely to be in treatment if they are infected or to be using pre-exposure prophylaxis if they are not, both of which significantly cut the chances that an infection will be passed on.

“There are just not enough services,” said Dr. Ronald O. Valdiserri, an AIDS expert at the Bloomberg School of Public Health of Johns Hopkins University, who wrote an editorial accompanying the study.

“It’s not just medical care — it’s housing, employment, vocational rehabilitation and transportation,” Dr. Valdiserri said. “These individuals are fairly vulnerable, and there aren’t enough assets to cope with their illness.”

The study, by epidemiologists at Emory University in Atlanta, was published online Tuesday in a relatively obscure outlet, The Journal of Medical Internet Research: Public Health and Surveillance. But it was accompanied by a second editorial by top AIDS specialists at the Centers for Disease Control and Prevention.

In South Carolina and Mississippi, the study estimated, 28 percent of gay and bisexual men are infected with H.I.V. In Louisiana, the estimated infection rate is 26 percent, and in Georgia, 25 percent. In Washington, D.C., which resembles a city more than a state in its demographics, it was even higher, 32 percent.

The lowest estimated infection rate was in South Dakota, where less than 4 percent of gay and bisexual men are infected. Among states that have particularly large gay populations, California’s estimated infection rate is 17 percent, Florida’s 18 percent and New York’s 20 percent.

The trend was even more pronounced in cities. Researchers counted only men who had received an H.I.V. diagnosis (the statewide figures relied on estimates), and consulted previous data on how much of each city’s population was gay and sexually active. Then the researchers calculated which 25 cities had the highest infection rates among those men.

In Jackson, Miss., that figure was 39 percent; in Columbia, S.C., 30 percent; in El Paso, almost 29 percent; and in Augusta, Ga., 27 percent. Twenty-one of the top 25 cities were in the South.

The lowest rate, just under 13 percent, was found in the Los Angeles-Long Beach-Anaheim metropolitan area.

Nationwide, the report said, about 11 percent of gay and bisexual men have an H.I.V. diagnosis.

“We hope these data empower local public health officials, community-based organizations and everyone fighting H.I.V. to bring resources to the gay and bisexual men who need them the most,” said Dr. Jonathan Mermin, director of the C.D.C.’s National Center for H.I.V./AIDS, Viral Hepatitis, S.T.D. and TB Prevention.

I will say that, at least in NYC, there has been a very noticeable surge in ads about HIV in recent months. Seems to be a good way to tackle the stigma and fight against fear with knowledge.

I met a lot of well off gay 20 somethings 5-10 years ago who didn't use protection under the assumption that they'd just get aids and take the medicine. I was like....what the fuck. You also lived through the late 80's early 90's and that's your attitude?
 
That seems crazy high. Really highlights the need for awareness. Sex education - of both young people and adults - is so damn important.

Most 20 somethings you met 5 - 10 years ago would have had no real recollection of what happened to adults during the last century.

Sadly, there are some guys out there who think it is an inevitability that they are going to catch it and if they do, they just take the meds and are done wit hit.

It's very sad to see guys who are 18, 19, 20 or so who are already infected with HIV. Yeah, there are medicine, but you are still living with a disease that has the capacity to kill you if you stop taking the medicine. Not to mention, PrEP and HIV meds in general are fucking expensive.

They were in the same age group as me and had the same scary as fuck sex education courses on the matter. AIDS was no joke back then.

I know in D.C. legislation was blocked that would have been able to combat this issue more effectively.

I wonder if it is the same for the other cities.

We have clinics in Houston that specialize it, and the Ryan White program will help anyone get the medication they need if they are infected. The medication cost over $2k a month. The copay would be anywhere from $70-$100, but the makers of the medication cover the copay.

It's a scary thought, but lots of people are still running around having bareback sex and justify it because they are on Prep or whatever. Still not a good idea.

I know about bug chasers, scary stuff. What was almost as scary was that these people weren't bug chasing, they were just being fuck all lazy and irresponsible. Oh, I can afford to pay for this chronic illness, might as well not be careful and just get it anyways.

What's more sad about this study is I just read somewhere that men who are part of the bear community tend to suffer from lower self-esteem and tend to engage in much riskier sexual behavior.

The problem is, alot of people are not confident, so they will say yes to bareback sex if it just means getting to have sex with someone. I don't really have any friends in the bear community, so I haven't had the chance to talk to them about issues like this.

I still think one of the most despicable things I've seen on the internet was a group of people who would purposefully get infected and then go around having sex with people who wouldn't know that they're infected.

All the gay apps out there are not doing any favors to the gay community in terms of keeping HIV infection rates in check. Not to mention there are people who are blatantly dishonest about their status.

Sure, it's good that HIV is no longer the death sentence that is once was, but people need to stop treating it like the common cold or flu. The virus is for life. It's still a virus with the possibility of mutating and be able to evade the current HIV drug to keep it in check. People are forgetting basic science when it comes to that.

Yep, that's why I'm on prep(prevents HIV aka Truvada) and get tested every few months. Even if I don't use a condom I still have some form of protection. HIV is no joke.

It seems like fucking an unending stream of uncaring strangers would be more monotonous than actually getting to know someone. But that's probably just me.
 
With the medical advances we have made can't people who are insured live a full healthy life with HIV? a friend of minds contracted Hiv about 17 years ago and is still alive and well. She has to take medication every day but if she didn't say she is HIV positive nobody would know.

Yes. It's considered a maitenance/long-term illness now if properly taken care of.

Of course, but sometimes it isn't an option and it's great that I have another layer of protection.

Be glad you can afford it. Not everyone can.

Clinics offer programs that help you pay for it or all of it in my case. I'm in Texas of all places and it's pretty easy to join the Giliad program.
 
DC's been an STD vortex as long as I've been alive. I think this area has the most STDs per capita of any city in the country. HIV alone has been a major issue here for at least 25 years, you'd think people would have figured out how to try to minimize risks by now.

I was just about to say this. It is consistently at the top of the lists for STD rates in the US. I don't know why. I guess it had to be somewhere

These medication have serious side effects. From premature liver failure, to eventual drug resistance by the virus. People should never even take the risk based on the fact that it's treatable. You have to have a really good insurance coverage if you think the average person could afford the treatment. (the pills cost between $2000-$5000 a month).

As someone who is gay, these numbers don't shock me at all. I live in a state where the infection rates are significantly less, but I know a lot of reckless guys who don't even consider using condoms. People they have sex with claim they are negative and go with it. Three of my friends were diagnosed with HIV the past two years alone with guys apparently who claimed to be HIV negative. It's spreading like wildfire in guys between 18 and 25. It's really sad, but I think education in schools and other institutes should spread awareness about this issue.

A lot of them were kids, you understand? That doesn't excuse them, but they probably don't remember the hysteria. It also doesn't help that they think they're invincible and/or they won't get it.

I know someone who is 51 years old, and lived through the epidemic in his twenties/thirties, but still does bareback sex. He showed me pics of friends of his who passed away because of AIDS. It's mind boggling, but sex can easily without doubt cloud people's judgment.
 
Contrary to what people may now believe, to many being Gay is still something to be utterly ashamed of, those in the closet or ashamed of themselves are far less likely to use contraception. This is especially true in poorer, usually predominantly black or latin, neighborhoods.

HIV is still an epidemic.

My doctor once told me about how some guys would meet to have a bareback orgy, one of them being HIV positive but no one else would knew who he was. A new kind of russian roulette it seems... some people enjoy the risk, scary stuff

The fact that we have effective drugs for aids likely increases the number of infected. It's not a death sentence anymore, so people aren't as scared. Plus they aren't dying out, so the population of infected just keeps growing.
