Metal Gear Solid (x7) - Back when I got my PS2 at launch, there weren't many new games to play so I heard MGS was amazing...and well it was.

Super Mario 64 (x5) - 12 years old and a N64, not much choice. $60 games and 6 month droughts of new games. Still love it.

Ico (x4) - Amazing game. Short, so it's easily beatable in one or two sittings).

Castlevania: Symphony of the Night
3 times, once on PS1, once on PC and recently on PSP.

The runner-ups are FF6 and Starcraft + Expansion with 2 times each. I usually don't complete single-player games more than once. I have played many western RPGs with more than one character/party, but not to completion.

I've also killed Diablo and his mean brothers in Diablo 2 and LoD about 2000 times with at least 20 different characters, but that's multiplayer.
 
Probably MGS 2, beat that every day for a whole week at one point. :lol

As for games with a playtime over 10h Final fantasy 7 & 8 and Baldur's Gate 2. at LEAST 3-4 times each.

*oh, and Star Ocean 3, about 4-5 times, goddamn trophys...

Ninja Gaiden Black is pretty amazing for me, I've been able to constantly beat it and then start it from the beginning again in a harder mode, still having lots of fun.

I think Mario bros on the NES....was able to beat it like under 5 minutes and can't really count how many times I beat it.

Metal Gear Solid on the psx is right up there, was totally hooked on it back then...think I beat it more then 10 times.

I beat Metal Gear Solid 1 und 2 over 10 times. And I mean every single game. It's such a pleasure to play them. Surprisingly i beat MGS3 only 5 times.

Secret of Monkey Island and Commander Keen 4.

Must be seven or eight times each for both of those; back when I had a 286 with EGA graphics, and there was nothing else coming out that would run on my computer.
 
Zelda Ocarina of Time : Between 30 and 40 times
Zelda Majora's Mask : Between 15 and 20 times
Zelda Oracle of Ages : Around 10 times
Zelda Link's Awakening : Around 10 times

Super Mario Land : More than 10 times
Mario 64 : 10 times or less

Kirby's Dream Land : More than 10 times
 
If beating Street Fighter with a single character and getting their ending counts then it's probably in the triple digits. Same for lots of other fighters.

MGS is probably:
MGS1- 10
MGS2- 5
MGS3- 7

Didn't Chrono Trigger have like 6 endings? I got them all whatever they were.

Old shooters like Gunstar and Contra beat dozens of times at least.
Lots more I can't think of.

Otherwise I'd be in the quadruple digits for SSB/SSBM

Super Mario World... easy since you can speed run in 30 or so minutes
Master of Magic... about twice. Awesome game, gimme sequel :)
Warcraft 3... the story doesn't get old
The Zelda double screen LCD game... :D
Gargoyles Quest... first real videogame I ever owned
Final Fantasy IV... the 2nd time on the GBA
Zelda, LTTP... 2nd time on GBA, again

Numerous Fighting games... yeah, listing each one individually would be too much

I'm sure there are some I have forgotten.

Vagrant Story at least 7 times ( I love this game ).
FFT twice.
FFVI twice.

FFXII twice jap and us.
Chrono Trigger twice.
Sam & Max, Day of the tentacle, Monkey Island 2 many many times ( I was young and bored then ).
MGS 1&2&3 about 4-5 times. 3hours~ speed run for MGS3.

I'm am forgetting some mario/zelda and few other games.

Edit I goddamn forgot RE4. So many times! at least 6 times GC and once PS2.

The original Pokémon games. I used to transfer my old Pokémons to my friends cart, then start a new save and transfer them back. :) I also completed the Oracle series countless times. More recently I played through Resi 4 three times and which is very unusual for me. It's just so damn entertaining and the fact that you got to keep your old guns adds to the fun.

I think anyone who is anyone who owned Sonic 3 or Sonic 3 & Knuckles felt the compelling need to make all 6 / 8 save slots complete with all Chaos Emeralds and Super Emeralds, and all the different variations, Sonic, Tails, Knuckles, Sonic & Tails...

I certainly did :D

Halo 1 - countless times going through the coop campaign with my roommate freshman year.
