This is clearly when the injury took place. Had nothing to do with Ryback.
I knew his heel turn would come back to haunt him.

First time Cena got hurt and needed surjury, Ryback got shoved in two straight main event feuds he couldn't creatively win. He went from an undefeated monster to losing 5 straight high profile matches.
Now, They turn Ryback heel and Cena gets hurt leaving a gaping hole in the top babyface slot Ryback could have taken. At this point I think Cena is intentionally ruining Ryback's career.

Yeah, I suspect that was selling. Cena may no-sell a ton, but that doesn't mean he can't. Sting was the same way, he no-sold a bunch of stuff, but sold like Flair vs guys like Vader.

I think this is a work.
He is ether only slightly hurt and WWE are doing the classic wrestling thing of making it sound worse then it is in order to make the wrestler look tougher OR the entire thing is fake and this is WWE being immature asshats and sticking it to Rock and other wrestlers who take time off because they are hurt.
