So it's apparently either/or with actions/design. In that case, I'm gonna go with Tomaru Sawagoe. Dude is arguably worse than Curtis Blackburn depending on who you ask.
First, the "lighter" stuff on his list of atrocities:
-He's a sadist, going out of his way to make people miserable.
-Doesn't have an ounce of empathy in his body.
-No remorse for his actions at all.
-Is a terrible parent.
That last one is important because "abusive parent" doesn't even begin to describe him. Spoilering because it's very disturbing.
The guy's family tree is so tangled that essentially everyone in the franchise are related. Pretty much every single one of the characters' problems can be traced back to him. Oh, and he never suffers for any of it. Also, he's immortal, so he can keep doing this forever.

God, where to begin. He's a dick to his male offspring, but he actually rapes the female ones. Oh, and it doesn't matter what age they are. Teenagers, preteens, toddlers; it's all fair game for him. To make matters worse, he impregnates most of them. As some are too young to get pregnant, he actually uses experimental drugs on them to fix that. There's something like three generations where he impregnates a 5 year old, then her 5 year old daughter, and so on. The guy is raping his descendants!
