Yeah, Bloodborne crows...They ended a no death run :'D They barely move, flail about linearly ,but they still killed me (at least)once XD

The torch hollow outside the bonfire in the Depths in Dark Souls.

I was farming humanity off the rats and to alleviate boredom I was getting a bit fancy. Took off most my normal armor and was wearing some light stuff, more a dress really, and seeing how fast I could complete a circuit.

As I started this next circuit, I swung at the hollow. And whiffed.

He proceeded to open a can of torch-pain on me.

The zombiemen in OG Doom- despite fighting wave after wave of Hellknights, Achviles, and Pain Elementals, all with ease, I've often lost my life to some faroff shotgunner who managed to take a potshot off me.
 
Any enemy in Dark Souls that has a dagger. They die in like two hits, but god help you if they hit you once, cause your getting stun-locked.

The old men in wheelchairs in Bloodborne. Ain't no disability in Yarnham.

Shotgun guys are way more dangerous than they seem at first thanks to being hitscan enemies and the fact that they don't command as much attention as the big guys so they tend to be ignored when they really shouldn't.

I was power leveling a friend at a Yuhtunga Jungle mandragora camp on Red Mage once in FFXI, and I forgot to use stoneskin before using convert, and died to a crit.
 
I'd say the Bloodborne crows can get you because of the horrible shriek they do. It's still quite alarming even after hearing it many times. Don't think I ever got used to that sound.

Dark Souls enemies can be so lethal (note: lethal does not mean the same thing as "hard") that just about any of them /can/ kill you if you fall asleep at the wheel. BB and DS3 are really the "worst" at this, focusing on increased lethality to boost difficulty. Note I put worst in quotation, as I don't really think of it as necessarily a bad thing just a design decision, so please spare me that faux outrage souls fanboys.

A bunch of deaths were caused to me in bloodborne because I got frustrated and let enemies waylay me to death that shouldn't have. Usually it's a sign to me that it's time to take a break for a bit more than anything particularly "wrong" with the game.
 
I got killed by a group of those Bunny enemies in Xenoblade Chronicles 3D.

In my defense I was underleveled a bit, I got rushed by like 5 or 6 of them at once, and the game isn't particularly built for group combat IMO.

That they are- did full Ultra-Violence runs of Doom 1 and 2 last year, and more deaths were at the hands of them than any Spider Mastermind. Thankfully with your own shotgun doubling as a sniper rifle, they aren't as annoying as some of the enemies mentioned in this thread.

Hell Vanguard from DMC 3. It's fine though, I got cocky with my skill, leading to a lack of oversight, and my eventual punishment.
 
Not a normal enemy, but Celestial Emissary in Bloodborne took me at least three tries due to getting too aggressive. I died less to Ebrietas and Paarl.

I've also died to Goombas, Bloodborne crows, and Castlevania skeletons.

God the room in the Undead Parish Church is full of them and they killed me at least 3 times

The first soldier in Goldeneye prooved to be very challenging for anybody unfamiliar with the N64 controller and 3d games in general. Half my class fell victim to it.

I was playing Treeline on League last night. My team got an ace after I barely survived a 1v2. I almost got away too, but a ranged minion struck me down.

On my first Dark Souls 1 playthrough, I actually died to the mosquitos in Blighttown, and to Pinwheel as well. Yes, I was terrible.
 
The fodder in Nioh aka the skeleton warriors. I got addicted to the sword skill Lai, while practicing I left myself quite open a couple of times and since Im a hoarder I did it while low on health because I didn't want to waste my elixirs lol

I died to the first zombie in Resident Evil.

I also died to the boulder in Ocarina of Time where you get the kokiri sword.

I died to each of these bosses 50+ times each, most I died to any bosses in any Souls game I played (demon's, dark and dark ii).

I beat Ornstein and Smough on my third try, and beat the Demon's Souls Maneaters on my first try. but the rats, bro
