He's made far more than 200. At one point his chipin donation alone was like 3k, forgot how much exactly. I think it was the last time he did run 12ish hour a day also.
Now he has subscribers money and ad revenue too.

No but he would have to wait until Honey or the other top two players beat his record so people would watch his stream if he continued to play 64, other than that he said he hated Sunshine unless he changed his mind.

He just said in his chat yesterday he barely makes any money off his stream. Maybe it was $2000. That still isn't much considering the ridiculous amount of hours he's pumped into the game.
He said he hated the other 3 last year, so ya he probably did change his mind about Sunshine. He played it a few times on the stream but went back to 64 right away. He played Ocarina of Time also so he can choose to do that.
Some chat users did a calculation last year that knew the payout of his ad service with the amount of people on, he is making decent money plus donations. He is having fun and getting paid at the same time so it's all good.
How much do twitch casters get per ad with like 3-4k viewers? although I'm sure tons use Adblock
No idea, sometimes ad blocking programs don't work with stream ads so people end up seeing them anyway.

I've literally never seen a twitchtv ad, so it probably doesn't apply to adblock which I imagine is the majority of what people use.

Yeah I think he realizes that people like dumping stupid shit in the chat, and it retains some viewers. He regularly tells his mods not to censor stuff unless it's outright spam, and he barely ever has slow-mode on.
