It was the fact that it's a sexualised imagination of a cartoon character that is more covered up than any other character in the actual media it belongs to....with proportions where her boobs are bigger than her head, while making her suck a popsicle that is also dripping on those boobs.

The clothing is stuff she wears in official concept art, in which her breasts are also quite large. The popsicle thing, well we know why that was done, but meh, still don't see it as creepy.

There's sexy and there's creepy. Not everything sexy is creepy and vice versa.
This one in particular is creepy to me because of the kind of character personality she has and the way she appears in the game.

But I can see where people are coming from, not everyone likes to see something sexualized like that, when the character in the game is much more innocent and is more "cute" than "sexy". I think that's why some people find it creepy, because it's a perversion of something that is otherwise quite harmless. Some of us like that, others don't. Live and let live, people shouldn't be laughed at or labeled as prudes just because they don't like something.
