It is a rare time that I get my full 120ish down from PSN. There isn't really anything you can do beside go wired to ensure you aren't facing any wireless limitations or issues.
Also don't get hung up on speed tests on the consoles. They can provide wildly different results and sometimes not even be close to what you are actually going to get. My Xbox One will show me 50ish but immediately download in the 90 range.

On my PS4, which connects through wire, It fluctuates between 250KB/s to 3-5 MB/s
Yup! Basically just letting you know you're not the only one with this problem. If you can get faster speeds on PSN I'd be very interested in hearing about it. I'm using ethernet as well.
I know it varies by area. I lived in Southern Alabama where the cable modem and dsl connection speeds were advertised capped at 10 Mbps and that was amazing.
I also lived in the panhandle of Florida (Pensacola area) where they had extremely fast cable modem service (Cox Cable) and even Verizon Fios.

I tend to get really slow speeds on PS4 downloads to begin with. As slow as ~250k/s, even. Over time, they speed up to a reasonable rate, but I'm lucky to approach 3.5mb/s, so consider yourself lucky anyway.

Yep, same here. I get 17mb/s on my phone, but am lucky to get 2mb/s on PS4. It makes playing online extremely infuriating. The entire PSN infrastructure is borked. Always has been, always will be. Are speeds equal no matter what server you choose? If I choose a server in New York, you're saying it will have equal speed as the server here in SD? Yeah you can't test to PSN server but you can test the pathway to the geographic location. If I'm wrong and you can get full 85mbps no matter what server you choose, then disregard.
Edit - I just ran speed test to Boston and I got 1/3 the speed of my speed test to San Diego.

For a service that people pay for, PSN seems to have poor download speeds.
I can max out my 500Mbit on Steam, it sure is something to see it download at ~60MB/s. They have good servers at least for me in Sweden.
