When we say "perfect game", I think we mean absolutely perfect for us. As in, that specific game you don't see a single absolute flaw in. And the gameplay, atmosphere, story, music are all perfectly fit to your preferences. And for me, there is one game that does it...
Dark Souls is the "most perfect" game I've ever played. The game design is perfect in my mind, the performance issues and inventory management are the 2 things holding it back from perfection.
Which Tetris are people even talking about? NES/GB Tetris? It was easily beaten by The Grand Master series IMO, but some people still prefer the old ones.

Made a battle system completely unique to the series and then perfected it at the 3rd game. (4 and 5 ruined it but I hear 6 is just as good as 3 though)

I've always said that Castlevania: Symphony of the Night is an example of video game perfection.
It's not even my favorite game of all time, but it is perfect.
