How do I beat Mia in madhouse mode? Holy shit I moved like crazy room after room and pumped her full of handgun bullets plus axe attacks and she STILL DOESN'T DIE!
Why alma is in the game? Why the attached harm works like usual two seconds after it was cut?
Why the main character has been knifed during the dinner scene and he keeps going just alright?

Speculation spoilers below
He's more than likely infected.
Block her attacks when you're able. Otherwise, just run. Block her way with doors and move to the other end of the room. Strafe around her if she closes the gap and hit her back, but block and keep strafing if she tries to spin around.
Except the hole in his hand is sealed up by the medicine immediately, before he had any prior contact with the mold. So the jury's still out on that one. He's probably infected, but that doesn't explain the hole, unless the herbs are cultivated from Eveline's mold (kinda like how the First Aid Spray was likely made from t-Virus samples).
I really feel like Lucas is the only one who was evil even before Eveline. Do we know what happened to him in the end? He supposedly entered the salt mine, but I don't remember encountering him.
Man how did Capcom make this game on a budget after they had to spend so much on a female rig so you can play as Mia
I've got mixed thoughts on Eveline. To be honest, I think she's kind of meh until the very last area.
I like how she's used there (there's a very well placed hidden scare I experienced in the hallucination house that got me pretty good that I feel most would miss). Well that and Eveline's Room in the Greenhouse, and the scare on the ship with the monitor when Mia is using the camera to see Ethan in the Mold and Eveline comes out when you look away is maybe the jump scare that most got me in the game.
For her own character I liked her presence in the final area... To be honest, I kind of wish they had done more of that. They only really take advantage of her in Eveline's Room on the second floor of the Greenhouse, and the ending area.
But I admit I felt a bit bad for her with her final line she says as the old woman, the whole fact it's the only line we hear from her grandmother self, how she's sobbing, the delivery of that line, the timing, and the line itself, "Why does everyone hate me?"
I think that one moment made me like her a bit moor and feel some empathy towards her, and highlights the fact that she is a little girl who went from like, 10 years old to 80 in three years, and she's still a kid inside, and one who's her whole life either been treated like an experiment or a monster.
I know it's cheesy, but so is the series. I don't know, that singular moment made me like her a bit more.

I was under the impression he was reporting to another company and not umbrella... but now I'm not sure anymore.
I think he was evil all along. Him trapping a 'friend' in the attic is a good indication. He even complains about the bad smell and some weird fluid (I assume it is body fluid from a rotten corpse) liking down. I think this happened way before eveline, when he was still a child.
I believe there's a note in his room about him basically killing someone named Oscar long before anything would have happened to the family. Along with some other weird stuff.
Ethan loses his hand almost immediately after having it stabbed and we don't use the hand that specifically during the first fight with Mia.
The screwdriver went right through his hand when she stabbed him with it, so deep it lodged itself into the wall. For all we know it went into the wound and came out the other side. Next moment we see Ethan with his hand, he's already been infected.

Honestly they probably didn't over do the Mia section with effort, look at her shadow when you have the machine gun equipped.
Dice honestly doesn't care and would use any excuse possible.
thats not confirmed at all. he didnt eat the food, we dont know if hes infected because hes not showing any symptoms
the healing is very likely coming from the medicine in the house. its strongly hinted at after jack cuts ethans leg off and gives him strong medicine. i believe ethan was surprised it healed that quick too

Also, do all unlocks carry over on all dificulties ? Wondering if I can grind the game a few times on easy/normal to unlock all the items like the running shoes, art of defense, x ray goggles etc and then play madhouse with them.

I'm starting to think the radio call you hear directly before the mines is made to throw you off. When the soldier says Lucas has been in contact with someone, it could be interpreted as either Umbrella or BSAA. It's hard to tell. I'm honestly starting to think that Chris Redfield has made his own Umbrella that counters the one in Umbrella Corps, judging by the blue logo on the helicopter.

Also what happens if you cure Zoe and not Mia? Does she stay behind at the burning pier doing dick all while you awkwardly boat away with Mia being pissed you didn't cure her?

Am i correct, there are only 2 endings? Save Mia or ZOE?
He definitely is infected. The Chainsaw Mia hallucinations are also a good indication imo.

And as it transpires, you can't save Zoe despite your choice, so there's not much point unless you want to see the later Mia fight.

I was super tired when I was playing through the game, so my memory on this stuff is all over the place.
That's a D-Series arm and head. Basically, they are failed fetuses infused with the fungus used as the basis for the BOW.
The BOW production method involved infection of a human embryo and cultivating it. There are many attempts, Series A through D. All failures.
Series E produced Eveline, the first success.
You need infected samples to create a cure, so they used the D-Series they had on hand from the shipwreck.
Later we use E-Series samples from the vial to create more E-Necrotoxin.
I seem to remember the Mia shadow looking quite good and different to Ethan's, unless I imagined it.
The language used during that conversation is maddeningly, unnaturally, deliberately unclear. We've been set up for a ton of community speculation and possible DLC expectation one way or another.
When can Ethan lose limbs?
I played through the whole game and it never happened.
Ethan loses his hand almost immediately after having it stabbed and we don't use the hand that specifically during the first fight with Mia.
