Pre-determined, sadly. The same goes for hair, lips, and skin colors.
This can be easily 'fixed' by mods later though.

More hairstyles/customization options can realistically happen as long as they're approved by Bethesda.

You can customize both, but no, you can't play both or switch at any point.
