The guy left/was fired from Konami, affecting the company's share value and some of you still think this is all a ruse? "Yes, let's lose money, trick our shareholders, annoy the medical community with the help of a real doctor just so we can do some viral marketing for a game already hyped."

Obviously GAF is in on the marketing campaign. Everyone with a "hideo kojima game" avatar is actually a Kojima alt account and the doctor is actually Kojima's old friend from high school and they planned this years ago, him becoming a doctor and publishing papers, the crazy head transplant thing, being interested in garnering attention and money and even being suing Kojima/Konami and being interested in how much the game would make.
You see, it's not for MGSV though. It's for Kojima's exit strategy. He did punched a producer in the face and insulted him over not getting a steak dinner in order to get fired after the game was done but not before one last crazy marketing stunt. He does all this, gets fans worked up, fires David Hayter, employs Keifer to voice 10 lines in the game, gets everyone invested in the game but BANG out of the medical field comes this doctor who looks like the one in game! He then sues (and wins, Doktor has ace attorney Phoenix Wright on his side) and then on top of Kojima's payment for the game he now has 1/3 of the profits from MGSV (Wright is really good) to split with his old buddy. Then Kojima and Dr. No ride off into the sunset on their gold plated yacht as David Hayter takes off the Jack Bauer mask and whispers "Kept you waiting, huh?" to the girl that Quiet is based on and then they all live happily ever after once they swap heads in OUTER HEAVEN.

CANAVERO rushes into the lobby. He pulls out a gun and threatens the receptionist.
CANAVERO: Where is he!? That bastard who stole my face for his silly game!
RECEPTIONIST: You're the doctor... from MGSV...
CANAVERO: No! I'm the real thing. I wanted to sue him but Kojima just laughed at me. Told me to "please understand" or some shit. [raises the gun] Now tell me where he is.
RECEPTIONIST: [hands him a key card] Top floor. Penthouse suite.
INT. ELEVATOR - DAY
The elevator overlooks Tokyo as it ascends. CANAVERO looks at his gun and steadies his nerve.
INT. KOJIMA'S OFFICE - NIGHT
CANAVERO enters what appears to be a luxury hotel suite. Brahms plays in the background. He finds Hideo KOJIMA sitting behind a desk, browsing NeoGAF and chuckling to himself. KOJIMA barely notices him behind the monitor.
CANAVERO: [in English] You think this is funny?
KOJIMA bursts out laughing. He's just watched SunhiLegend's A Beautiful Mind gif. He finally sees CANAVERO.
KOJIMA: [in English] I didn't think you'd be here so soon.
CANAVERO: I'm a man of my word.
KOJIMA: That you are. I take it your little publicity stunt ruffled quite a few feathers?
They smile at each other. At the same time, they reach for their necks. They pull at what appears to be loose skin. Their faces contort and the skin is revealed to be a face mask.
Two ripped masks fall to the floor. They were disguised as each other, and now the real KOJIMA remains standing in front of the real Doctor CANAVERO at the desk.
KOJIMA: I guess you could say they riked it.
CANAVERO: [standing up]. Here, sit. The internet has made quite a fuss while you were gone. Preorders have skyrocketed.
KOJIMA instead paces to an enormous window overlooking Tokyo.
KOJIMA: I'll trust your judgment. I never was good with numbers. It'll be enough to cover your science project, I assume?
CANAVERO: Yes. More than enough.
Pause.
CANAVERO: What happens now?
Long silence. KOJIMA turns to face a company portrait on the wall. He sees all of Konami's big wigs standing and facing the camera like a school photo.
His eyes focus on President Takuya Kozuki and gradually narrow. Cut to black.

Two year timeline is a joke. An operating theater with hundreds of staff simultaneously is a joke. Performing spinal reattachment surgery in an hour is a joke.
