So let's assume for a second this were actually true. What could we do about it? Obviously blowing it up to smithereens would be impossible, but could we theoretically alter its trajectory if we used all available nuclear armament on earth?

Best method for redirecting incoming masses is to send out something to gravity tractor it out of the path of the earth

If we go that realistic, we'd probably notice a planet coming a good while away (the gravity something that large emits would not go unnoticed by our equipment on Earth). If it is on a collision course, you'd probably plan ahead with the hope of sending larger objects (meteorites?) in it's path and using that to change it's course slightly. I doubt our nuclear arsenal could detonate it away but using it to change course of meteors might work out better.

Course we'd have to find these meteors nearby on top of that which we haven't done succesfully yet.

It doesn't matter who we are, what matters is our plan.
I mean, the law of averages says one of these wild theories has to be correct.
 
Hypothetically if something similar ever happens. What are the measures that can be taken to prevent it?

Nothing, at the moment. The only thing we could do is look at fleeing earth.

The asteroid in the impact that we suspect ended the dinosaurs was only 10-15km in diameter, that left a 180km wide and 20km deep crater. Assuming impact, it would be permanently devasting for a planet sized body. We'd probably become another asteroid belt.

If the body manged to get locked due to gravity with earth, that could also be devastating because it would change tides and even the centre of gravity between the two like, say, Pluto and Charon, meaning the barycentre no longer lies within earth, potentially enough to remove the spinning of the core which provides the magnetic shield protecting us from the harmful solar winds, if that happens, our atmosphere would be completely stripped resulting in harmful radiation hitting the planet. Could also potentially tidally lock us to the sun meaning one side would be in permanent light, one in permanent darkness if there was any impact on rotation.
