Phase 4 marks the final beta of FFXIV: ARR. All progress in this beta will be carried over into the main game unless a major bug appears that forces the team to wipe character data. Phase 4 is an open beta of sorts, as anyone who signed up or participated in a previous beta is expected to be guaranteed access. Sign ups have since closed. Soon after phase 4 ends, "early access" will begin for pre-ordered and legacy (subscribed for +3 months) accounts.
Beta Phase 4 begins August 17th - UNLESS you participated in Phase 3, then it begins on the 16th - and ends on the 19th. (3-4 days)
Early Access begins on the 24th and ends on the 26th. (3 days)
FFXIV: ARR launches on August 27th, worldwide, for both Windows and PS3.

Pre-ordering the Windows or PS3 version will net you a code for early access and a few vanity items (assuming some underpaid teenager didn't screw you over at your GameStop).

Durandal - Japanese, Legacy (i.e. carried over) server. A linkshell (an invitation-based chat channel) for friends and GAF members. Speaking Japanese highly recommended, but not required.

4. Complete the form as required
5. Once approved you will get access to the forums and other areas

Common courtesy: If someone has already claimed an avatar (or has made a similar one on their own), don't use it or one that is a slightly modified version of it. (A good way to check if the one you want is taken is to skim a few pages of this thread.)
(I made a lot of avatars with transparent background. If you'd like you could use these to make your own avatar, even combining some of the avatars below.)
If you want to rep one of the three Grand Companies / Nations, check out these icons done by Ebil (he also did the flag avatars below): (These simply fit over any avatar you'd want to use. Feel free to scale them down or up.)
