You shouldn't speak with authority on a subject you obviously have very little knowledge of.
There are two common forms of SRS (sexual reassignment surgery). There are other techniques , such as where the colon is used to produce a self lubricating vagina cavity, but the two following are the most common from what I understand.
The most common is penile inversion, where the penile tissue is used to form the cavity and clitoris and the scrotal skin is used to form the labia. It does NOT result in a "small dot and nothing else", but rather a decent looking vagina with slightly funny looking labia that don't "sit" right (for lack of a better term) in my opinion. I believe this is the technique you see most often on Thai women.
The second technique, which usually has a much more aesthetically pleasing result is basically just the opposite. The penis is used to form the external parts of the vagina (inner and outter labia, clitoris, hood) and the scrotal skin forms the internal cavity. The results from this technique can be virtually indistinguishable from a natal vagina. For an example you can look up post-op pictures of transsexual porn star Danielle Foxxx. She went to Dr. Suporn (the man who invented the technique in Thailand) and had excellent results.
After surgery the woman must dilate her vagina (insert and apply pressure to what is essentially a dildo) for an hour or two every day for the first 6 months to a year to help the vagina keep it's shape. After that bi-weekly intercourse is usually enough to keep it from losing depth. The down-side with the second technique, while aesthetically more pleasing, is that the dilation procedure is much more intensive. I believe as much as 4 hours a day at the beginning.
The best and worst SRS surgeons in the world are in Thailand. Dr. Suporn pioneered the second technique I mentioned and you can look at his work for an idea of what is possible. American surgeons, while not as good as Suporn (again, in my opinion) often offer acceptable results but at a much higher price. There are other doctors in Thailand who do it for a much lower price but the results are fairly poor.
Results of course vary from individual to individual but patients usually remain orgasmic and often cite increased sensitivity.
Prices range from $6,000 at the lowest to upward of $20,000. Insurance does not usually cover this cost despite the fact that GID (gender identity disorder) is a recognized condition.
