Can someone explain the fuss over testing kits? Can't tell if it's a legit concern or just more political nonsense.

It's probably mostly politics; we're in an election year in the US. There's billions of $$$ floating around looking for oppo research to sell. Any aspect of this crisis is going to be used as political ammunition.

More testing would probably be good; however unless we're proposing testing absolutely 100% of the population it isn't going to change much. Enough people are asymptomatic that just testing the sick isn't going to tell you who all is carrying. Testing the sick at least tells you what treatments to use on them, based on what comes back from the test.

Haiti bans flights from Europe, Canada and Latin America

Haiti is suspending flights from Europe, Canada, the Dominican Republic and Latin America for the next two weeks during the coronavirus pandemic, according to Haiti's Prime Minister Joseph Jouthe.
Flights from the United States will still be permitted, "after an agreement with American authorities," Jouthe tweeted Sunday.
The tweet continued:
"All flights departing from Europe, Canada, Dominican Republic and Latin America are prohibited, for the next 2 weeks, from midnight on 3/16/2020. Only flights from the USA are still authorized, after an agreement with the American authorities," the tweet read.

Haiti has not yet recorded any confirmed cases of the novel coronavirus.

I think Gyms can also be meet and greet places for politician and business types in a huge city like NYC, might have a home gym too but this is PR for them.

New conference for The Dutch , I hope we close public transport and non essential shops and businesses, than we have a change . Full lock down.

We could be in the clear in 3,5 months here in The Netherlands.

Odds are it's contact with someone who didn't yet have symptoms. That's the bit that's hard to guard against and that's the bit that social distancing sorts out.

I am in complete agreement. Also, Dr. Michael Osterholm said that the virus is airborne. This is a piece of crucial information that was not known previously.

So basically nothing, still. It's alright saying people should stay at home for 14 days, but when there's uncertainty about pay if you have to have the time off then I would imagine most people will just continue to work if there is someone in their family with a mere cough. From my own experience most people still believe that 'its just a cold' and been blown out of proportion anyway.

I dont think they've ever said children can't contract it, just that it has been more mild.

Though it seems to be mild for almost everyone under 50 really, unless there are preexisting conditions.

As far as I know we haven't gotten age data from most of the countries for the deaths outside China and Italy.

How long are they contagious?

We really don't know yet! But patients can shed RNA from 1-4 weeks after symptom resolution, but it is unknown if the presence of RNA equals presence of infectious virus. For now, COVID-1 9 patients are "cleared" of isolation once they have 2 consecutive negative RNA tests collected 24 hours apart.

Vitamin C (Paraphrased)?

I have seen no real data on effectiveness of vitamin C and Vitamin D. I don't think it can hurt but there is no evidence to support that it will help you.

Asymptomatic spread (Paraphrased)?

I absolutely agree there is a tremendous number of people walking around with virus unknown to them. That is why the cases will rise rapidly as we are seeing but the mortality rate will decline.

Socialized Medicine to blame in Italy (paraphrased)?

I completely agree! We are much better prepared then people are giving us credit for!

My biggest fear is socialized medicine and having this kind of crisis. It's not surprising for those of in healthcare to see the higher fatality rates in countries with socialized medicine.

I am confident that the United States rates will drop significantly as the denominator grows.

That should not be considered risky. Use drive through at restaurants and pharmacy's to lower your risk.

CBS News spoke to one of the country's top experts on viruses, Marc Lipsitch from Harvard University, who cautions that 40-70% of the world's population will become infected and from that number, 1% of people who get symptoms from COVID-19, the disease caused by the coronavirus, could die.
Lipsitch: So say that's half the people. Then the 1% or 2% that we're seeing in the symptomatic people is cut down by half. Whatever the number is, it's gonna take a toll. If it really does spread as widely as that projection says, and that's what I think is likely to happen, then there are gonna be millions of people dying. And I don't think there's any way to get around that. And so I think there's real reason for people to be concerned. I also think that we can turn that concern into actions that will make the situation better.

It's really impossible to tell at this point. But be prepared because the media will go after every doom and gloom story they can find.

But also remember that the Flu kills well over 500,000 annually. So if we and other countries don't take serious precautions than a million plus deaths is not unreasonable.

We all of the things we and other countries are currently doing, I think it would be unlikely to see that type of outcome.

But as the numbers start rising quickly the more people get tested, CNN will find every guy they can that will tell them"WORST CASE SENARO". I tell my patients the media makes money from fear not good stories so don't watch.

For instance, I could save 10 lives today but wouldn't care or report on it at all. But if a doctor down the street had a patient die from a mistake it will be the lead story.

Statement:

People who keep asking me about the numbers being so low that is because the majority of people are completely asymptomatic and not tested. In addition somehow the media is not getting all of them either. But they are being reported to the appropriate authorities.

For instance this story has not been in the news yet but probably will. I know of a physician who tested positive. He was and still is asymptomatic. However, he and many other physicians who he contacted are now in self isolation. This creates more burden because it decreases are number of providers. In addition they are contacting recent patients he saw to let them know to isolate themselves and watch for symptoms. It does not mean they will all get the virus but they need to stay away from others until we know they do not convert to positive.

This will continue to happen as more patients and unfortunately even providers are identified with the virus.

As far as airborne goes I have heard that people often mix up what airborne means and that there hasn't been anything found that the virus is airborne.

What has been proven is that its respiratory which means it can remain in the air after you sneeze, cough etc, and test have shown it can remain long.

But when people think airborne they usually think its floating in the air, getting in AC systems etc.... This has not been proven. So the virus won't just circulate in the air on its own.

Its basically droplets vs aersol.

Well, at the end of the week, we will have good indicators where it's heading in all our nations.

So far, very few deaths in Sweden, despite a large active spreading of the disease.

Considering us lucky so far, we're 7 days behind Italty.

Italy declined. 3.200 today. Looking good.

Good for maybe flattening the curve, but can't take much from this yet. It can ebb and flow by a few cases each day. Deaths went from 250 to 175 back up to close 400 and now to 360 so a drop of a few hundered in cases doesn't mean that tomorrow it will go down again.

But if the curve stays somewhat static it should be taken as a good sign. If you look at China's numbers you can see it took about 3 weeks for cases to start to drop and about 4 weeks for deaths since the Hubei lockdown on the 23rd of Jan. (ignore the 14K case spike on Feb 12th, that was just a CCP dump).
 
22K in a day would be a stretch I think. If everyone was fully testing maybe.. I think we see 12K new cases maybe. Hard to tell though.

USA might have a spike if the drive-thru testing and other things do what they say they will do. So numbers could really go up a level or two.
We really should expect US to beat Italy without being too shocked if we were testing as we should.

This tidbit of information comes from another doc on the thread I referenced, but the main doc has said he is hearing the same things (though there currently is no research to reccommend)

Tylenol is recommended over those pain relievers/fever reducers that are processed through the kidneys. My family uses Aleve to treat normal aches/pains/headache. Is there a reason to switch to Tylenol prior to getting sick with CV?

Ok... So this one is a little medically complicated but I will try to explain it so it makes some sense. The short answer to your question is maybe and it is a theoretical and anecdotal benefit more than a study-proven benefit to take Tylenol over NSAIDs (ibuprofen (aka Advil/Motrin) and naproxen (Aleve)).

There is some evidence that this virus may bind the ACE receptors (Angiotensin Converting Enzyme receptors) in the body as a way to get into cells. By a complicated pathway, NSAIDS increase the number of ACE receptors in the body and so there has been a recommendation that we give acetaminophen (Tylenol) instead as it does not up regulate ACE receptors in the body. The thought is that you could potentially make a person worse with NSAIDs by unintentionally increasing their ACE receptors binding sites for the virus.

Since body aches and congestion are much less intense in Covid-19 and the presenting symptoms are usually cough and fever anyway, I think it is reasonable to avoid NSAIDs for now and treat fever with Tylenol if you are worried about Covid-19.




Basically don't use NSAIDs as they may worsen the disease. Apparently Doctors in Italy have found this out as well.

they are making corrections all the time with US from all tracker sources, Most countries release new cases numbers at one or two reports per day but the States release them at all hours of the day it makes it hard for the trackers to keep up on who released what.

My State tends to push cases to New York for example, tested here and works here but lives in NY the case is moved. Different things like that shift numbers.

Straight from the Doctor.

Absolutely no steroids if you can avoid them. The studies in China did show a correlation between death and steroid use. Now take in to consideration over there most of the population smokes so the steroid use could mean they had severe COPD but regardless it weakens your immune system so if you can be off them I would recommend it. Unfortunately some people need steroids.

As for aspirin/NSAID's they also should be avoided. We don't want any drugs that could affect the kidney's as the two most common causes of death are respiratory and renal failure.

Nothing against you and your contributions. But this is pretty annoying and contributes nil at this point. The numbers are there, and still comings; the virus has made its point in abundance. These constant jabs accomplish nothing, whether they come from you or anybody else. I suggest we focus on the matter at hand, that’s all.

President Trump urged governors on a call today to try to secure additional medical equipment on their own without waiting for the federal government to intervene, though he did say they would try to help.

"We will be backing you, but try getting it yourselves," Trump said on the call, according to a person familiar.

He was discussing what health experts say is a serious need for more respirators and ventilators to deal with the influx of coronavirus patients.
