If you're facing the door Hemwick is to the left. The one to the right is a dead end for now, but you might want to go a little bit in there just to take care of two annoying enemies that won't respawn back.

Go beat the boss of Old Yharnam, and then a door will open in the room with the Cathedral Ward lantern which will eventually lead you to the other side of the closed gate.

kill the Blood-Soaked Beast, go to the Healing Church Workshop, make you're way down the silly falling on platforms section and go into an elevator that takes you into the Inner Cathedral Ward.

Hammer is nice but very slow so I would say saw blade.

Not bad. There was a good Amazon deal going on, but you might not be able to take advantage of it depending on where you live. That's still a pretty good price if you aren't paying a lot in sales tax. That's roughly $470 USD? That's outright buying the console and the game at full price.

I think 95. I got tired of the last boss, so I went out grinding for about a day and went back to it. Levels make a huge difference in bloodborne.

You can upgrade a lot of weapons to + 3, the mats are easy to come by. So if you like using it, go ahead and upgrade.
