You are correct, Sigma Octanus IV was in July. Halo Waypoint confirms Reach fell on August 30th, 2552.

By general Sci-Fi cannon I think it generally is named after the star and then the planet away from the star. So if Earth were a simple mining colony and we were from another star system, Earth might be called Sol III. That is not strictly adhered to, but it seems to be only the more important or more inhabited worlds that have proper names like Reach (centre of UNSC military forces) or Harvest (a veritable breadbasket of the human colonies).
