Gal Pani X. It's a Japanese PC freeware game. You can get it here:

It's so addictive... the name suggests that it was inspired by Gals Panic (a series I didn't hear of until long after I'd first played this game), but really it's its own game, with some major differences from other games in the genre. The game has some significant differences from other Qix games, but if it has to be compared to what I know of Gals Panic, which isn't too much, Gal Pani X is more like the later, anime-style ones, but better, I think. It's got anime art of characters from To Heart (mostly, perhaps all). There's no nudity, but it probably is NSFW due to the more suggestive second stages in each level.
The biggest difference between Gal Pani X and most any other Qix games is that you aren't stuck just moving around the outline of the area you have filled in this game, like you are in most Qix games. Instead, you can actually move around freely within the filled area. This doesn't mean that the game is easier, though -- instead, it's actually perhaps harder, because the game mixes a whole bunch of bullet-dodging into its Qix. You need to be able to move around fully within the filled area, you'll need the space to dodge all the waves of bullets, missiles, and more that the enemies shoot at you... the only almost bullet-hell-style Qix game? As far as I know, yes.

The mix works very well, and results in an addictive and compelling game.
It really gives the game a very different feel from other games in the genre, and sort of spoiled me for them honestly... I mean, I do like the whole genre, but Gal Pani X is definitely my favorite. I've played a good bit of it over the years, really. I still can't beat challenge mission 2, though, because I've never quite been able to figure out the combo system. Oh well...
It's a challenging game, but it's got a good difficulty curve -- it starts out easy, and gets more difficult as you go, and as you try to master the game and learn its depth. Oh, it is in Japanese only, but the English menu option translates most of the relevant text, so it isn't really an issue.
-The game has branching paths, and you choose which level to go to after each level in arcade mode.
-You play a second, harder stage in each level if you 100% clear the first one. There are different endings (different credit sequences, pretty much) depending on how well you do.
-Good graphics, catchy music, good presentation for a freeware game
-Challenging gameplay that scales up as it goes. It starts out quite doable, but at its hardest is very, very difficult. The difficulty curve, and rewards for playing better, are done very well.
-Unique mix of classic Qix wallmaking and shmup-style bullet dodging that works well
-Lots of unlockables, hidden options to unlock, gallery mode images to get, etc -- it'll take a long time to get everything in this game. The "turn all hitboxes bright red" option that you can unlock particularly makes a real difference. It makes the graphics look awful, with all the sprites covered in red boxes, but it makes precise bullet and enemy dodging much easier and simpler. No more guessing about where, exactly, you can touch.
I admit, the To Heart characters did get me interested in playing it at first, along with the Qix gameplay, but it's the great, and original, gameplay that kept me coming back.
The same person behind Gal Pani X has only made one other game I know of, the pretty good, but extremely difficult, Gauntlet style game Sispri Gauntlet (that is, Gauntlet with Sister Princess characters). It's fun, but I'd like it so much more if there was saving, and continues... as it is it's insanely hard! I don't usually get past level two, and there are at least eight levels... and I'm not bad at Gauntlet. It's also available at the site I link above.

There's always Cacoma Knight for SNES. Fun little game
That's a fun one too. We need more Qix games, it's a pretty good game concept...
Really, about Cacoma Knight, my only major complaint is that the game is quite easy. For anyone with much skill at all in Qix games, you won't have much trouble beating the game. I definitely had fun while it lasted, but only the last stage was actually difficult; I did get game overs there several times, before I finally finished the game. Overall though, it's easy. Fun, but easy. Still, if anyone who likes Qix finds a copy for cheap, sure, pick it up. It'll be entertaining for a little while. Amusing little story, decent graphics, fun gameplay... the randomness of the treasure chest locations is a little annoying, but overall it's good. I just wish there was more to it.

Woah, that does look amazing. Great sense of style, and it looks like good, solid Qix gameplay, too. Looks like great stuff...
