There's definitely room for improvement with PAIN, but it's pretty fun. I've already put a few hours on it. Really addictive, but it seems like a demo. Needs more levels and custom soundtracks.
I'd pay $30-40 for a really fleshed-out, polished version.

Having actually done this sort of work on a different game, I can tell you the size difference is usually related to localization in Europe, where there's a lot more languages you have to worry about. Specifically, the speech audio can often add up to a few hundred MB.
