For PC they just send you a link saying the code is in your Amazon "game library"

I have not, but just FYI that another option would be to buy PSN codes (or I assume the same for Live) on Amazon, then pre-order via PSN. That way you'll be able to pre-load. Amazon probably won't let you pre-load, plus they may charge you tax depending on where you live (they won't charge tax for the PSN codes and PSN won't charge tax when you order there either). Saves you a little $.

I imagine so. I'm not sure when the embargo ends, but you would imagine up until release to prevent campaign spoilers. If you see other people streaming then go for it, but I wouldn't risk being first to try it.
