Our brains have an innate sense of the sex characteristics the bodies they inhabit are supposed to have. Outside of that core of gender identity, in a world where there's no societal pressure on gender expression, that may be all it is. Not to downplay the effect that those influences have in the present, because that reality is almost certainly unachievable. As it stands there's a lot of influence from societal expectations and pressures conflicting with people's sense of selves.
