Not sure about the latter two, but during the mission briefings you have to bump into those you want the facecamo from. So if you want Otacon's camo, you have to bump into him (with the Mk.II).

Yeah, got it when I went for the Chicken Emblem. Also, walk up to a guy using that camo, they freak out.

To get them to do the crawling pose I just lay down on the ground and roll over onto my back and they'll get down on all fours. Pull your camera out and you can stand up and move around and they'll stay in the same position as long as you don't put the camera away. This only works in the dream world though. You can get them on all fours in the regular level but they won't pose for you.

You cannot use the camera during cut scenes.

And to use it in thrid person, select "2p mode" with the camer and use R1 on controller 2 to snap pics.
