The TV broadcast of episode 18 of Psycho Pass. While the art and animation wasn't always perfect in that show it was at least serviceable...not this episode. We're talking mouths not syncing, frames of animation completely absent bad plus some really off key art. I can't be bothered to hotlink images but there examples in this thread behind the spoilers.

I'm also being told episode 4 of Gurren Lagann also had issues with it's TV broadcast version but they were not as bad as this.

Would Urusei Yatsura movie 2 count as infamous too? While contemporary viewers and fans see it as the best Urusei Yatsura movie; at the time, the author of the manga nearly rejected the script and the fans took a disliking to it (possibly because it can be viewed as a critique of the repetitive nature of the anime series and those fans who lock themselves into it's routine) which the director; Mamoru Oshii took the flak for. As a result it was to be his last work on Urusei Yatsura (directed the first film and had various roles on the TV series).

This a thousand times. The show's not exactly happy (despite that really cute and peppy opening song), but this episode is super fucked up compared to the others.

Even the manga was controversial when Dark Horse and other Western editors like Egmont Manga edited that scene

You are probably right (been a while). I guess I remembered the manga version which I saw randomly when I looked for information about the censorship of a different manga.
In the manga, "to finger someone" gets a whole new meaning.

single father of the main character dies from getting hit in the head from a baseball. But not only that everything seems fine after the hit so try go home and go to bed. He then passes away in the night. The mc being 5 things he's sleeping and won't wake up so he goes about his morning.
Super fucked up and gets me every time I see it.

Yeah, that was a heartbreaker.

Major is a masterpiece in general.
Wouldn't call it a masterpiece but it's definitely one of the better baseball anime.

beheading
of the confident, experienced mentor character which signals a massive change in tone towards what the series is really about.

This episode also is the first to feature the ending theme and animation which is dark as fuck in contrast to the bright and colourful opening that persists throughout the series

Facade? The first episode
had the cuddly cat mascot being shot to shit and the second had a woman trying to commit suicide.

Well magical girl shows have a tendency to be really dark anyway, but they usually don't go all the way dark like Madoka does

Can't remember specific episodes so I'll just say Gundam Seed Destiny in general, for me personally

I thought Seed was alright as the first Gundam show I watched to the end, but Destiny was so shit and pissed me off so much that I didn't want to bother with any anime for the next three years. I remember that the final episode of Destiny was so badly rushed they apparently released a different 1-hour version of it later, but it was still bad

Never seen any of the Monogatari series (Bake, Kizu, Nise), but that one made the rounds of memespace well enough.

The third episode of Yoake Mae Yori Ruriiro Na with the infamous QUALITY cabbage that had to be reanimated in later iterations:

I forget the exact episode, but Naruto vs Pain fight had one particular episode where they tried some experimental animation style and it absolutely flopped and looked HILARIOUSLY bad. I didn't even watch the show at the time, but me and my friends all had to queue up the episode and we nearly died laughing.

What's a shame is that Shikimaru+Kakashi vs Hidan+Kakuzu had that same sort of hyper fluid offmodel art and animation style expect that it actually ended up looking really good. I don't know what went wrong here.

Ash's most recent failure to capture a Pokemon League win is a good candidate for this. The fact that so many people actually bought in and thought that Ash was gonna turn it around after damn near 2 decades of being a goober is sad, but the resultant GAF salt thread was incredible.

Also, I just want to remind Hyperactivity and SalvaPot again that they STILL owe me those Best Wishes reviews from our bet.

What show is this?

"Madoka Magica" aka the best magical show around. It completely deconstructs the Magical Girl formula so it's not lot Sailor Moon, Pre-Cure, Magical DoReMi, Winx Club, etc.

Iggy getting his ribs kicked but still managing to use the last of his energy to save polnaref was heartbreaking

I remember having watched the OVA first and they made this scene much more brutal. Poor Iggy...I was shocked despite reading the manga prior to that.

Uglier version of Iggy is cut to half by Vanila Ice's Cream Stand. Then a furious Vanilla Ice stomps over and kicks Iggy's upper half!

Whatever episode the bath scene was in; Kill la Kill. It didn't bother me much but I remember there being a ton of negative reactions.

Now I recall, Mawaru Penguindrum episode 8 is very infamous. Well, almost every episode is infamous, but this was something else when was broadcasted.
Thank god Ringo grew up and she turn the best character in the show.
