The only thing I can defend about American cultural values when it comes to violence and nudity is that they are just that. Values. They are not government controlled. It's a sort of quasi-censorship. We are capitalists, so we want to make money. If you are HBO and have a show with gay porn and you stick it on right after Flight of the Conchords people will either be mad because their child could possible find it or people might just not like gay porn. They won't subscribe to your service and you are screwed.
Same with networks like Comedy Central that run South Park. At first it's a hard show to get on the network, because they fear people won't watch. The network knows how the public react in America to dirty language. The execs worry that advertisers won't pay for spots without an audience (and they are right). In the end money makes this country go round.
One might say that Rockstar is brave for putting a dick in a game played mostly by dicks. We have already seen some people lash out in homophobic protest over this, so we will see if they made a good move.
Sadly the quasi-censorship comes back into play when the news outlets pick up the story of a penis in a game. The vocal minority start calling for the game to be pulled from shelves (the stores choice of course), or the ESRB to re-rate the game. The ESRB fears bad PR and caves under the pressure. In the end you don't even need an Orwellian government to censor since we already have the idiots to do it for them.

The USA does not have a government mandated age for purchase of M rated games, so a kid can still get a hold of this game in some places one would assume. Until we do these people are not going to stop, and to some people they have a leg to stand on.
